Applied Root Cause Analysis (ARCA)

Introduction

Root Cause Analysis (RCA) is a method that is used to address a problem or non-conformance, in order to get to the “root cause” of the problem. RCA is used to assist in identifying additional barriers and items to correct or eliminate the cause(s) of a safety event and/or a non-conformance with regulations or standard as a result of an audit. With a comprehensive RCA process “real fixes” that address the “real issue(s)” help in preventing recurrence of the safety event or audit finding. Without a comprehensive RCA process, it is likely the “true” problem will not be addressed (or addressed in full), and a similar safety event or audit finding will reoccur – representing wasted time, effort and money to an operator. 


This three (3) day Applied RCA course is structured to allow for discussion and hands-on exercises dealing with how to address audit findings and/or investigation of safety events; and how to consider the possible Corrective Actions to these. The course also covers the use of root cause analysis for failure analysis in maintenance and for systems analysis to support change management and risk.

  • Root Cause Analysis (RCA) and the Corrective Action Programme
  • Overview of RCA – Process; Use – including macro-level for trend analysis, senior management briefing 
  • Conducting RCA - including overview of ICAM
  • Common tools and methodologies used - e.g., 5-Why, Fishbone, MEDA, etc.
  • Root Cause Analysis Exercises
  • Human Factors for RCA
  • Intervention Design
  • Corrective Action Plans
  • Issue Close Out (Verification and Validation)

Who should attend?

Individuals responsible for or participating in investigating reported safety, quality, and security events; and auditing findings so as to recommend corrective action. Individuals responsible for planning, directing or managing a safety programme and supervisors who are required to supervise an accident prevention / risk management programme. 

Personnel from civil aviation authorities responsible for undertaking any form of regulatory audit or inspection activity and regulatory staff at all levels who are responsible for planning and managing oversight activities will benefit from attendance at this course. Those who conduct investigations into safety events; as well as review proposed corrective actions from clients.

Prerequisites

A background in aviation, with an exposure to Human Factors and Safety Management/Quality Management concepts. Should also have knowledge of RCA and conducting RCA.
